Please note the following before completing your Notification form:
- Notification forms cannot be submitted directly through the website to the Customer Service Centre. Forms can be submitted as outlined below.
- Payment is required at the time the notification is filed.
- For credit card payments — please contact the Customer Service Centre by phone: 1-877-372-7233.
- For cheque or money orders — download and complete the applicable form, and mail it to:

ESA has a program for acceptance of retrofitted luminaires in Ontario – click here to view this guide. A notification of work is required for each project site with retrofitted luminaires. Please note a completed application form must be submitted to process your request. An application cannot be completed over the phone. |

Plan Review Forms
As of September 15, 2021, ESA no longer accepts email, fax and paper submissions. All submissions must be made using the Electronic Plan Review (EPR) Portal.
To be eligible to use the EPR Portal, you must have a valid ESA customer account. For assistance in creating an ESA customer account and/or to register to use the EPR portal, please contact the ESA Plan Review department at 1-800-746-6480 and dial “0”.
Please note: being a user of the LEC Portal does not grant access to the EPR Portal. If you currently use the LEC Portal and are also a Plan Review customer, you will need to register for use as noted above.
