Request for Inspection	To request an inspection on an existing notification of work

Please note the following before completing your Notification form:

Notification forms cannot be submitted directly through the website to the Customer Service Centre. Forms can be submitted as outlined below.
Payment is required at the time the notification is filed.
- For credit card payments — please contact the Customer Service Centre by phone: 1-877-372-7233.
- For cheque or money orders — download and complete the applicable form, and mail it to:

Residential

New Residential	For new installations on a residential premise (includes: modular/prefab homes, multi-unit residential complexes [row-house, townhouse, semi detached house, duplex, triplex and quadruplex])
Residential Renovation	For renovation of individual residential units, including multi-unit buildings consisting of 4 or fewer units, basement apartments, granny suites, detached outbuildings and rewires
Apartments	For new installations (new apartment building/retirement home) and renovations of apartment units, multi unit residential buildings (with common entrance) 5+ dwelling units and retirement home
HVAC Small Job	For HVAC equipment installations done at Residential and Apartment sites.

Industrial/Commercial/Agricultural

High Voltage	For high voltage (primary) installations >750 Volts (Temporary and Permanent)
Low Voltage	For low voltage (secondary) installations <=750 Volts (Temporary and Permanent)
Renewable Energy	For renewable energy (solar, wind, other) installations 10 kw or less
Neon Sign	For inspection of the wiring of signs & outline lighting installations (includes skeletal neon, outline lighting, and signs with remote transformers or ballasts)

Entertainment

Permission to Show/Energize	For trade show exhibitors with unapproved equipment that wish to display but not to connect or provide electricity to the equipment or for trade show exhibitors that wish to connect and energize (provide electricity) unapproved equipment.
Entertainment	For entertainment notifications/permits including all television, film, commercial, live performance, carnivals, trade shows, or event productions including but not limited to live productions regardless of site or location.

Record Search	This service generates a report for a specific address indicating if there are open/outstanding notifications of workpertaining to the address. If copies of correspondence, notes and/or specific details about any notifications (open or closed) for an address are required, please submit an Access to Information Request.
Access to Information Request	A Request for Access to Information is broader in scope than a Record Search and may involve specific information and notes recorded in a notification of work, and/or copies of ESA correspondence. Please note a completed application form must be submitted to process your request. An application cannot be completed over the phone.

ECRA/ESA Electrical Contractor Licence Application	Application Form and process on how to apply for an ECRA/ESA Electrical Contractor (EC) licence.

Linking multiple accounts with an Electrical Contractors Licence	Fill out this form to link multiple ESA accounts for the same business under one Electrical Contractor Licence (owned & operated under the same name, in different locations).

Serious Electrical Incident Reporting	Form used by local distribution companies (LDCs) to notify ESA of any serious electrical incident.

Substation Maintenance Condition Report	Form to be completed in its entirety, within 14 days of shutdown, for all substation maintenance jobs & all situations where equipment replacements (equivalent rating/characteristics) are completed.
Low Voltage Service Maintenance Condition Report	Form to be completed in its entirety, within 14 days of shutdown, for all low voltage service maintenance.

As of September 15, 2021, ESA no longer accepts email, fax and paper submissions. All submissions must be made using the Electronic Plan Review (EPR) Portal.

To be eligible to use the EPR Portal, you must have a valid ESA customer account. For assistance in creating an ESA customer account and/or to register to use the EPR portal, please contact the ESA Plan Review department at 1-800-746-6480 and dial “0”.

Please note: being a user of the LEC Portal does not grant access to the EPR Portal. If you currently use the LEC Portal and are also a Plan Review customer, you will need to register for use as noted above.
